Science Corner 71 | Does Caffeine Help You or Hurt You?

Caffeine is the most widely used psychoactive substance on the planet, and most adults take it daily without a second thought. But scan the headlines from the past year and you'd think we still don't know if it's safe: heart palpitation warnings one week, brain protection claims the next. Both can't be the whole story.
Before we get into the research, it helps to know what's in the cup. Caffeine content varies a lot by drink, even among drinks that look identical:
Brewed coffee (8 oz): about 100 mg
Espresso (1 oz shot): about 64 mg
Decaf coffee (8 oz): about 2 to 15 mg
Black tea (8 oz): about 30 to 60 mg
Green tea (8 oz): about 25 to 50 mg
Decaf tea (8 oz): about 1 to 8 mg
Matcha (8 oz): about 60–70 mg
Most healthy adults can handle up to 400 mg a day, roughly four cups of brewed coffee, without issue. Keep those numbers in mind as you read what follows.
The largest, longest-running study on caffeine and brain health ever conducted just tracked more than 131,000 people for up to 43 years. It found the opposite of a warning label.
The 43-Year Study Behind the Headlines

Researchers pooled two of the longest-running health studies in the country: the Nurses' Health Study (86,606 women, 1980 to 2023) and the Health Professionals Follow-up Study (45,215 men, 1986 to 2023). Published in JAMA, the analysis followed participants for a median of 37 years, checking coffee and tea intake every two to four years with diet questionnaires.
The results are hard to dismiss. People in the top quarter of caffeinated coffee drinkers had 141 dementia cases per 100,000 person-years, versus 330 cases in the bottom quarter, an 18% lower risk overall. Higher intake also linked to a 15% lower rate of subjective cognitive decline, and in the women's cohort, better scores on objective memory tests. Tea drinkers saw similar patterns. Decaf drinkers did not, which points to caffeine itself as the active ingredient, not just coffee's other compounds. The benefit peaked at a moderate range: about 2 to 3 cups of caffeinated coffee a day, or 1 to 2 cups of tea.
One caveat: this is an observational study. It tracks existing habits rather than assigning people to drink coffee or not, so it can't prove caffeine causes lower dementia risk. But a sample this large, followed this long, across two separate cohorts, is about as strong as observational evidence gets.
It's Not Just Brain Health
The dementia data aligns with a much larger (and consistent) body of research on caffeine and longevity overall. Even dating back to 2015, research found that drinking up to 4 cups of coffee a day was associated with a 14% lower risk of all-cause mortality and a lower risk of cardiovascular death, compared to not drinking coffee at all.
Timing adds yet another layer. A 2025 study followed over 40,000 adults and split coffee drinkers into "morning-type" and "all-day-type" patterns. Morning drinkers had 16% lower all-cause mortality and 31% lower cardiovascular mortality than non-drinkers. All-day sippers drinking the same total amount didn't show the same benefit. When you drink your coffee may matter almost as much as how much.
Two people can drink the exact same two cups of coffee and walk away with completely different experiences, because they are, biologically, running different software.
Why Your Caffeine Tolerance Isn't Your Friend's
Most of caffeine's breakdown, roughly 95% of it, happens through a single liver enzyme called CYP1A2. A common genetic variant splits people into two camps. Approximately 43% of people carry a version that clears caffeine quickly, while the remaining 57% carry a version that clears it more slowly. That genetic split helps explain why caffeine's half-life varies so widely from person to person, generally somewhere between 2 and 14 hours. Age and hormones shift this further. Caffeine clearance drops by roughly a third between early adulthood and your mid-60s to 70s.
It's very important to note that the metabolism of caffeine is far from just a comfort issue. Some research links slow metabolizers who drink a lot of coffee to higher rates of hypertension and heart attack, an association that doesn't show up in fast metabolizers at the same intake. The same cup of coffee is not the same exposure for every person drinking it.
Timing Is Its Own Variable
Caffeine's average half-life is 5 to 6 hours, but it ranges from 2 to 10 hours depending on who's drinking it. A randomized trial found that caffeine consumed even 8 hours before bedtime measurably disrupted sleep, cutting total sleep time by 45 minutes and decreasing sleep efficiency.
Sleep researchers generally recommend a cutoff of 8 to 10 hours before bedtime, with slow metabolizers needing more of a buffer. Paired with the morning-versus-all-day mortality data above, the pattern holds: caffeine's effects depend on the clock, not just the dose.
When Caffeine Isn't the Right Call

The protective story above applies to generally healthy adults at moderate intake. It doesn't apply evenly to everyone.
Pregnancy: Guidelines from the American College of Obstetricians and Gynecologists recommend staying under 200 mg of caffeine a day. Intake at or above 300 mg a day has been linked to higher rates of pregnancy loss and lower birth weight.
Anxiety and panic disorders: Caffeine can trigger palpitations, jitteriness, and panic symptoms in people with anxiety or panic disorders, even at normal doses most people tolerate comfortably. Researchers are actively studying this link in people with diagnosed panic disorder.
Heart arrhythmias: Very high intake, generally above 400 mg in a short window, has been associated with irregular heart rhythms and elevated blood pressure. Anyone with a diagnosed arrhythmia should talk to a cardiologist before making caffeine a habit.
Certain medications: Some antiarrhythmics and psychiatric medications interact with caffeine and can amplify side effects. This is a conversation for a prescriber, not a guess.
Slow metabolizers with existing cardiovascular risk factors: The same coffee habit that looks protective in a fast metabolizer may carry more risk for a slow metabolizer who already has hypertension or heart disease.
What This Means for You
Across all of this research, a few practical patterns hold up. The sweet spot looks moderate: 2 to 3 cups of caffeinated coffee, or 1 to 2 cups of tea, is where the protective associations are strongest and most consistent across studies. More isn't automatically better.
Why less tea vs coffee? The benefit peaked at a moderate range for each drink on its own: about 2 to 3 cups of caffeinated coffee a day, or 1 to 2 cups of tea. These aren't directly comparable by caffeine content, tea carries its own mix of plant compounds that may contribute independently, and the two beverages were analyzed as separate habits, not as equivalent doses of caffeine.
Earlier in the day is generally better than later, since morning-concentrated intake tracks with better mortality outcomes than spreading the same amount across the day, and caffeine within 8 to 10 hours of bedtime can measurably disrupt sleep.
Finally, your own reaction to caffeine is worth paying attention to. If one cup leaves you wired or an afternoon coffee keeps you up at midnight, that could be your genetics at work.

I go through caffeine like it's a job requirement, and I still don't really like coffee. Never have. We've got a two-year-old, so sleep in this house is a moving target, and some mornings I just need it to work, full stop.
Caffeine is personal for many people, and there is a lot of experimentation that people use in order to find their proper dose and timing. My overall recommendation is to be patient with it, go slowly, and simply enjoy the ride. Coffee, tea, matcha, and many other formats have deeply personal relationships for a lot of people, and that is okay. At the end of the day, you need to do what works best for you.
As a final note, please remember that caffeine is a dangerous substance. You can overdose on caffeine, and it can be very uncomfortable When you get above recommended limits, be smart about your dosing and even then be slow when you try to increase.
